Financial Policy

  • As a courtesy we will bill your primary and secondary (if applicable) insurance companies. You are responsible for any deductible, co-pay and/or any balance due after insurance has processed your claims. Your insurance is a contract that exists between you and the insurance company. Please see insurance information below. If there are any questions about your bill, it is important that you communicate with us as soon as possible. Please contact our office to discuss a mutually agreeable payment plan that will not jeopardize your credit. If you do not have insurance, we will require a deposit on account at the time treatment is rendered.

  • Please advise the receptionist of any changes that may affect your billing: ie changes of employment, address and/or new insurance.

Motor Vehicle Accidents

  • If you are involved in a MVA, we will bill the auto insurance. You are responsible for keeping us informed of any decision or changes that could affect the outcome.

Missed Appointments

  • A 24 hour notice for any appointment you cannot keep is appreciated. A$50.00 charge will be incurred if you fail to notify us or cancel your appointment within 24 hours.

  • No Shows: If you miss an appointment and do not call to notify us or make arrangements, we reserve the right to cancel all future appointments and may require that you call to schedule any further visits.


  • We value your time and don't want to keep you waiting, but occasionally, we are delayed by an unexpected event with another patient. Please be assured that the quality of your care will not suffer.

  • If you arrive late, your treatment will end at its scheduled time in order to avoid delays for patients after you.

Patient Responsibilities

  • Request additional information if you do not understand something.

  • Inform your therapist if you anticipate problems complying with the treatment plan.

  • Demonstrate respect and consideration to other patients and facility staff.

  • Participate in the development and implementation of the plan of care.

  • Notify your therapist of any changes in your condition.

  • Patients who smoke: Kindly refrain from smoking for 1 hour prior to your scheduled appointment. Please ask if you have any questions regarding this.

What to Expect

Patient Forms

At South Whidbey Physical Therapy in Freeland, our treatment emphasis is on supervised exercise and education. The ultimate goal is for you to return to normal activities of daily living, work and participation in sports or recreational activities.

  • Visits will last approximately 45 minutes to one hour, during which you will work with your own personal therapist.​
  • Your therapist will answer all of your questions related to what activities you should or should not partake in during your rehabilitation.
  • You may be given home exercises to assist you in working toward your treatment goals.
  • As you improve, your exercise program will be progressed to fit your needs for coordination, retraining motor patterns, balance, endurance, speed and strength training.
  • We can assist you with the transition from exercise for rehabilitation to exercise for health maintenance, weight loss, cardiovascular improvement, return to work and even high level athletic performance.
